Subject: Revised Mandatory Monitor Review Form
Effective: June 1, 2001


This Policy Alert replaces Policy Alert CACFP DCH 2000-1.

Special Nutrition Programs (SNP) has revised the Form 1607, Monitor Review, and instructions for use in monitoring the participation of day care home providers in the CACFP.

The revised format for Form 1607 permits sponsors to electronically or manually pre-print selected elements on the Form 1607 prior to conducting the monitoring visit. The revised instructions identify which elements may be electronically or manually pre-printed. The revised format also provides the option for listing the names of children by observed attendance or by enrollment.

You may begin using this form upon receipt. However, you must use this form without modification to conduct monitoring visits on or after June 1, 2001.

Note: You are not permitted to use or develop an alternate version of Form 1607.

SNP has enclosed a camera ready copy of the Form 1607, with instructions. Sponsors may reproduce the form as needed.
